Eligibility
Membership is valid only at Happy Harbor Pediatric Dentistry for services provided by our team. -
Non-Transferable
Membership is non-transferable and cannot be shared or gifted to another patient. -
Non-Refundable
Membership fees are non-refundable, even if services are not used during the plan period. -
Coverage Period
Membership is valid for 12 months from the date of purchase and must be renewed annually to maintain benefits.
- Exclusions
Membership does not cover: -
- Treatment completed outside of Happy Harbor Pediatric Dentistry
- Products (toothpaste, toothbrushes, etc.)
- Care referred to a specialist outside our office
- Discounts
Membership discounts apply only to services completed during active membership. Fees for treatment are due at the time of service. - Missed Appointments
Standard cancellation and no-show policies apply to all appointments under the membership plan. - Changes
Happy Harbor Pediatric Dentistry reserves the right to adjust membership fees or terms annually.
